Patient Registration Form »Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid as per the contract agreement. We work with most Insurance companies and will do our best to file your claim for you. You will be responsible for any balance not taken care of by your insurance policy. As a courtesy to our patients, we will bill insurance companies for services provided. If you have any questions, our knowledgeable staW is always available to answer them. Insurance policies vary, so we are available to review your insurance with you to determine the appropriate course of action. Once treatment has started, we will file your claims. To help us with your insurance filing, please bring your current insurance information to your appointment.
